The Health Coach agrees to provide one-on-one health coaching services, which may include advice, education, and support regarding chronic conditions such as diabetes, hypertension, cholesterol, obesity, and overall wellness. These services are provided through telephone calls, email, and/or video conferencing.

1. Scope of Practice

The Client acknowledges and agrees that:

• The Health Coach is not a licensed medical professional.

• The Health Coach is not providing medical diagnosis, treatment, or prescription services.

• The Client is responsible for seeking medical advice from a licensed healthcare provider before making any changes to their health regimen.

1. Client Responsibilities

The Client agrees to:

• Be truthful and open about their health history, conditions, and goals.

• Follow the coaching plan and take personal responsibility for their health decisions.

• Consult with their physician or healthcare provider before implementing any health advice.

1. Confidentiality

The Health Coach agrees to maintain confidentiality of the Client’s personal health information. However, if required by law or if there is a risk of harm to the Client or others, the Health Coach may disclose certain information.

1. Liability Waiver

The Client acknowledges that:

• Health coaching services are for educational purposes only and that the Health Coach is not liable for any injury, illness, or harm resulting from the Client’s actions based on the advice given.

• The Client assumes full responsibility for their health decisions.

1. Payment & Fees

• The Client agrees to pay the Health Coach for services rendered according to the agreed-upon fee schedule.

• Payments are due prior to each coaching session or as outlined in a payment plan.

1. Termination

Either party may terminate this Agreement at any time by providing written notice to the other party. No refunds will be given for services already rendered, but unused sessions will be refunded at the discretion of the Health Coach.
