Terms and Conditions of Service RenewUrHealth

These Terms and Conditions govern the use of services provided by RenewUrHealth, operated by Guerdy Surphin, Certified Health Coach. By enrolling in or using RenewUrHealth services, the Client agrees to the following terms.

Nature of Services

RenewUrHealth provides preventive health coaching, lifestyle education, and population health support services. Services may address nutrition, physical activity, behavioral change, weight management, stress reduction, and support for chronic conditions including diabetes, hypertension, hyperlipidemia, obesity, and cardiometabolic risk factors.

Services may be delivered via telephone, secure video conferencing, or electronic communication.

Not Medical Care

RenewUrHealth does not provide medical diagnosis, treatment, prescriptions, psychotherapy, or medical advice.

Clients must consult a licensed healthcare provider before making changes to medications, diet, exercise, or other medical decisions.

For medical emergencies, call 911.

CDC Program Participation

If enrolled in the CDC Diabetes Prevention Program, attendance, weight measurements, and related participation data will be collected and submitted in de-identified format to the Centers for Disease Control and Prevention in accordance with DPRP Standards.

Hospital and Clinic Collaboration

When services are delivered under a healthcare partnership, RenewUrHealth operates in collaboration with referring providers but does not replace medical oversight. Services are provided under applicable contractual agreements when required.

Client Responsibilities

The Client agrees to:

Provide accurate information
Participate actively
Take responsibility for personal decisions
Seek appropriate medical care when necessary

Results are not guaranteed and depend on participation, consistency, and individual circumstances.

Fees and Payment

Fees are due according to the agreed schedule. Services may be suspended for non-payment. Private coaching services are subject to agreed pricing at the time of enrollment.

Cancellations

Sessions require 24-hour notice for rescheduling. Missed sessions without notice may be forfeited at the discretion of RenewUrHealth.

Confidentiality

All personal information is protected under the RenewUrHealth Privacy Policy and applicable regulations. Please refer to the Privacy Policy for details regarding data protection practices.

Limitation of Liability

Health coaching services are educational in nature. RenewUrHealth is not responsible for outcomes resulting from personal decisions, failure to follow medical advice, or misuse of information provided during sessions.

Termination

Either party may terminate services with written notice. Refund policies, if applicable, are governed by the agreement in place at the time of enrollment.

Governing Law

These Terms and Conditions shall be governed by the laws of the state in which RenewUrHealth operates.
