These Terms and Conditions govern the use of services provided by the Founder of RenewUrHealth. By enrolling in or using RenewUrHealth services, the Client agrees to the following terms.

RenewUrHealth provides preventive health coaching, lifestyle education, wellness guidance, and population health support services. Services may address:

• Nutrition 
• Physical activity 
• Behavioral change 
• Weight management 
• Stress reduction 
• Lifestyle improvement 
• Support for chronic conditions including diabetes, hypertension, hyperlipidemia, obesity, and cardiometabolic risk factors 

Services may be delivered via telephone, secure video conferencing, electronic communication, or approved digital platforms.

RenewUrHealth does not provide:

• Medical diagnosis 
• Medical treatment 
• Prescriptions 
• Psychotherapy 
• Emergency care 
• Individualized medical advice 

Clients must consult a licensed healthcare provider before making changes to medications, diet, exercise, or other medical decisions.

For medical emergencies, call 911.

If enrolled in the CDC Diabetes Prevention Program, attendance, weight measurements, and related participation data may be collected and submitted in de-identified format to the Centers for Disease Control and Prevention in accordance with DPRP Standards.

When services are delivered under a healthcare partnership, RenewUrHealth may operate in collaboration with referring providers, clinics, hospitals, health plans, or community organizations, but does not replace medical oversight. Services are provided under applicable agreements when required.

The Client agrees to:

• Provide accurate and complete information 
• Participate actively in services or programs 
• Take responsibility for personal decisions and lifestyle choices 
• Seek appropriate medical care when necessary 
• Maintain respectful communication 

Results are not guaranteed and depend on participation, consistency, effort, and individual circumstances.

Fees are due according to the agreed schedule. Services may be suspended for non-payment. Private coaching services are subject to pricing in effect at the time of enrollment or purchase.

Scheduled sessions require 24-hour notice for rescheduling when applicable. Missed sessions without notice may be forfeited at the discretion of RenewUrHealth.

Personal information is handled in accordance with the RenewUrHealth Privacy Policy and applicable privacy regulations. Please review the Privacy Policy for full details.

Health coaching and educational services are informational in nature. RenewUrHealth is not responsible for outcomes resulting from:

• Personal decisions 
• Failure to follow professional medical advice 
• Misuse or misinterpretation of information provided 
• Delays in seeking emergency or medical care 

Either party may terminate services with reasonable notice. Refund policies, if applicable, are governed by the agreement in place at the time of enrollment or purchase.

These Terms and Conditions shall be governed by the laws of the state in which RenewUrHealth operates, unless otherwise required by applicable law.
